REQUIRED MASSAGE FORMS
All new Massage Clients must fill out the massage intake form prior to receiving services. When you book your appointment you should recieve a link in your confirmation email to complete our online intake form.
If you are not able to complete the online intake form you may choose to download & complete the PDF form below and either email it to us at relax@justbebodyworks.com or print it out and bring it with you to your appointment. We will also have a paper intake form available to complete at your scheduled appointment if needed.

Just Be Bodyworks LLC will occasionally request permission to take and use photos in sessions to use for promotional purposes.
